Health, Insurance and Private Clinics in Mallorca

Public healthcare, private health insurance, clinics and emergencies for residents, second-home owners and international patients.

Mallorca has a well-developed healthcare system with two clearly separate paths: the public system via IB-Salut and private doctors, clinics and insurance companies. This overview is a practical orientation for property buyers, residents, second-home owners and international patients. It does not replace medical, legal or insurance advice.

Public and private healthcare

The public healthcare of the Balearic Islands is organised through the Servicio de Salud de las Islas Baleares, IB-Salut. Those who are legally entitled in Spain gain access via the tarjeta sanitaria individual, the Balearic health card. It is the proof for public centres, hospitals, prescriptions and digital health services.

In Mallorca, the main public hospitals include the Hospital Universitario Son Espases and the Hospital Universitario Son Llàtzer in Palma, the Hospital Comarcal de Inca and the Hospital de Manacor. Allocation is generally based on place of residence, medical urgency and referral.

Private healthcare works differently: appointments are often available more quickly, many providers work in multiple languages and bill either directly to private insurers or to self-payers. However, private treatment is not automatically covered by the public system, the European Health Insurance Card or a foreign statutory health insurance fund.

EU citizens, EHIC/TSE and temporary stays

For temporary stays, the European Health Insurance Card, in Spanish Tarjeta Sanitaria Europea or TSE, can enable medically necessary treatment in the public system. It is important to distinguish: the card does not replace travel health insurance, does not cover planned treatments and does not apply if the habitual residence is moved to Spain.

Those wishing to be treated with EHIC/TSE should explicitly ask for public care: asistencia sanitaria pública or Sistema Nacional de Salud. If a practice, hotel doctor or clinic demands advance payment, it is often private care; such costs are generally not reimbursable via EHIC/TSE according to EU guidelines.

Residencia, S1 and private health insurance

Anyone establishing their habitual residence in Mallorca must clarify their insurance cover properly. Employees and self-employed persons who are socially insured in Spain generally enter the public system via the Spanish Seguridad Social. Non-working EU citizens usually need sufficient means and a public or private health insurance covering Spain for registration as a resident.

For pensioners from EU/EEA countries, Switzerland or the United Kingdom, the S1 form may be relevant. It is issued by the responsible state and registered in Spain so that the person receives healthcare in the country of residence. Whether an S1 is possible depends on the pension and social insurance status.

Non-EU citizens applying for a residencia no lucrativa must, according to official migration information, have a public or private health insurance taken out with an insurance company authorised in Spain. In practice, this means policies should be checked carefully before application for waiting periods, exclusions, co-payments, pre-existing conditions, inpatient benefits and coverage throughout Spain.

Convenio especial as a special path

For persons living in the Balearic Islands but without any other entitlement to public healthcare, the convenio especial de asistencia sanitaria can be an option. IB-Salut requires, among other things, residence or registration in the Balearic Islands, a previous effective stay and monthly contributions. According to official information, contributions are 60 euros per month for persons under 65 and 157 euros per month from age 65.

Private clinics and international patients

Mallorca is geared towards international patients. Larger private providers mention multilingual care, international insurance or special patient services. These clinics are particularly relevant for self-payers, privately insured persons, second-home owners and patients who prefer English or German-speaking care.

Before private treatment, patients should clarify whether the clinic bills directly to the insurance, whether prior authorisation is needed, what co-payments apply and whether emergency care, diagnostics, surgery, aftercare and medication are billed separately. For international policies, it is also important whether Spain is covered as country of residence or only as travel destination.

Emergencies in Mallorca

For acute emergencies, 112 applies in Mallorca as in the whole of Spain. The medical emergency service SAMU 061 coordinates out-of-hospital urgent and emergency medical care in the Balearic Islands. In life-threatening situations, one should not search for insurance details but call the emergency number immediately.

For less urgent complaints, the correct path depends on status: residents with an IB-Salut card contact their Centro de Salud or the public emergency structure; privately insured persons check the emergency numbers and contracted clinics of their insurance. Tourists and second-home owners should have their EHIC/TSE, ID, insurance documents and medication list ready.

Second-home owners

Owning a property in Mallorca alone does not entitle one to public healthcare. Decisive factors are residence status, habitual residence, social insurance, pension status and insurance evidence. Those only on the island temporarily should combine EHIC/TSE with a robust travel health insurance. Those living long-term or predominantly in Mallorca should clarify before moving whether IB-Salut, S1, Spanish social insurance, convenio especial or private full insurance is the appropriate path.

Good preparation is particularly important for chronic illnesses, regular medication, planned operations, pregnancy, care needs or existing diagnoses. In such cases, those affected should speak with their insurance, treating doctor and, if necessary, the Spanish health centre or clinic in advance.
